Supportive Housing Program
Supportive Housing services are available to residents in Jefferson, Lewis and St. Lawrence counties. In order to be eligible, an individual must be at least 18 years of age, have a primary diagnosis of serious mental illness (SMI) and experience substantial impairments in maintaining safe and secure housing.
Supportive housing is a person-centered housing model to assist individuals with safe and affordable housing options and to increase housing stability. An individual is linked with a Housing Case Manager who helps them address barriers and identify additional solutions and supports. The Case Manager supports individuals by developing a person-centered Housing Support Plan, notifying them of their tenant rights and responsibilities, helping them with the set-up of an apartment, and on-going rental support according to program guidelines. Additionally, the Housing Case Manager refers the individual to additional supports to maintain housing stability and further develop skills to live successfully in the community.
To enroll in the program, a Single Point of Access for Adults (A-SPOA) application must be completed and sent to the SPOA Coordinator in your county. You can make a referral for yourself or a family member. Individuals must meet all eligibility required to be considered for enrollment.
