Community Based Services

Delivered within an individual’s home or within the community, our community-based programs in Jefferson, Lewis, and St. Lawrence Counties are designed to support individuals with mental illness and/or substance use disorders achieve progress towards their goals as developed in their treatment plan. Outreach, engagement, and care coordination are critical components of these service offerings.


Child and Family Treatment and Support Services (CFTSS)

Lewis, Jefferson, St. Lawrence County

Psycho-social Rehabilitation (PSR) Services are a community-based services where a qualified trained provider meets with the child on a regularly scheduled basis to work on determined goals set by the youth and the family. These meetings are normally held in the community in locations the child is comfortable. The provider keeps in regular touch with the family and collaterals and holds meetings to determine progress towards the goals developed in the treatment plan. With time spent in the program the youth will build skills to help them overcome their personal challenges and the attain the goals they have set. When the child and family determine the set goals have been attained and satisfactory progress has been made the youth will be ready for discharge.

The CFTSS program offers the service of psycho-social rehabilitation (PSR). These are rehabilitative services (skill building) that can be delivered in the home or in the community.

PSR services are unique in that the delivery of services are in the community individualized to meet the child’s identified goals in locations that meet the child’s needs and utilizes their strengths as assets in the process.

Home and Community Based Services (HCBS)

Lewis, Jefferson, St. Lawrence County

The Home and Community Based Services (HCBS) Waiver respite program is designed to give a child and family a break from their daily routine in order to reduce stress and maintain family harmony in the home. These services are accessed through a Health Home care manager.

Hourly overnight respite is offered as well.

Medicaid eligible or enrolled individuals and those who meet health home care management criteria are eligible for these services.

Opioid Overdose Prevention Program (OOPP)

Lewis, Jefferson, St. Lawrence County

Our Opioid Overdose Prevention Program (OOPP) is registered with the New York State Department of Health (NYSDOH) and it allows us to train and provide Naloxone to non-medical persons who may witness or experience an opioid overdose. Naloxone (commonly referred to as Narcan) is a medication that can reverse the effects of opioids and prevent death from an overdose. These programs aim to reduce the harm and mortality associated with opioid use by increasing access to Naloxone and educating the public on how to recognize and respond to an overdose.

There are currently over 1,000 registered opioid overdose prevention programs in New York State, including community organizations, health care facilities, pharmacies, law enforcement agencies, and others. OOPPs like ours can order Naloxone kits at no cost from the NYSDOH or the NYC Department of Health and Mental Hygiene (NYC Health Department) and dispense them to their clients, patients, staff, and community members. They also provide training on how to use Naloxone safely and effectively, as well as information on opioid addiction, treatment, and recovery resources. Outreach and Offsite Services

Home visits and offsite services are a critical part of our care management, peer, and outreach services. By engaging individuals in their homes or within the community, our qualified staff are provided with critical insights relative to the level of functioning and any barriers to treatment including food, transportation, housing, safety, education, etc. Working collaboratively with individuals in the community, our staff aim to provide comprehensive care coordination and linkages to necessary supports that will aid them on their journey to improved health, recovery, and wellness.

Furthermore, our outreach and offsite services aim to engage and increase access for individuals who may be reluctant to receive onsite services at our clinic or for those in hard-to-reach locations. Examples of in-community services provided by our agency include those within schools, at jails or prisons and probation; within drug court and opioid court; at community-based organizations; and within the Department of Social Services. Delivery of these services could include mobile clinical services, telepractice, and/or peer services.

In addition to these services, we offer appointment reminders, transportation coordination services, vocational education services, health home care management services, harm reduction education, insurance navigation/enrollment support, etc. Our interdisciplinary team coupled with strong community/regional partnerships allow us to provide comprehensive wrap around services to support the mental, physical, and social needs of individuals.

Respite Care

St. Lawrence County

Respite care is available in private homes for children who live in St. Lawrence County. This program is open to all children. Referrals are made by the local mental health agencies and/or other social service agencies.

County respite services are designed to provide a child a break from the home to improve family harmony.

Single Point of Access (SPOA)

Lewis, Jefferson, St. Lawrence County

Single Point of Access (SPOA) is operated by local county government units to enable adults and families easy, streamlined access to the mental health service system regardless of their financial resources or insurance status. While SPOA does not provide any direct services, the SPOA team can provide support and guidance to navigate the local system of care. SPOA services are intended for individuals who are at high risk of psychiatric hospitalization and/or who are unable to maintain community-based linkages and supports such as psychiatric treatment, medication management, medical treatment, housing, substance use treatment, legal, and other social supports. The goal of SPOA is to have the individual receive the appropriate amount of support at the right time and at the right place. The county SPOA offices accept provider and self-referrals. The SPOA Coordinator gathers information by reviewing the referral forms and conducting intake interviews with the individual(s), then they present the information to the SPOA Committee.

The SPOA Committee consists of representatives from area service providers. Meetings are held at least once per month. The Committee reviews the individual’s intake information and determines eligibility for services and appropriate level of care. The individual(s), and any representative at their discretion, are encouraged to attend and actively participate in the meeting. Through this process, community-based service providers are identified and assigned to assist the individual(s) in addressing identified needs.
